Now What?"},"content":{"rendered":"<p><br \/>\n<\/p>\n<div dir=\"auto\">\n<p><span style=\"min-width:0;\" data-state=\"closed\">Sara Szal MD<\/span><span> <\/span><\/p>\n<p>I had real information for my patient.<\/p>\n<p>She was 34, sitting across from me with a stack of labs her doctor told her were normal and a fear that no reassurance to date had resolved.<\/p>\n<p>Her mother\u2019s fertility ended early, without warning, the type of family history that you wonder about. She wanted to know if the same biological clock was running inside her, the one her gynecologist, mom, and group chat all read differently. She couldn\u2019t afford to freeze her eggs. She wasn\u2019t in a relationship stable enough to plan around. She was anxious about what to do next.<\/p>\n<p>Lab tests like anti-M\u00fcllerian hormone (AMH), estrogen, and follicle stimulating hormone (FSH) can tell you if ovarian reserve is declining early, but they cannot give you an exact date. <\/p>\n<p>She was not sick. She was doing everything that a responsible 30-something is told to do: eat a lot of greens, lift heavy, track the cycle. Some nights she asked ChatGPT and Claude the questions she was afraid to ask out loud. <\/p>\n<p>While the lab tests could tell her something true, they could not tell her everything.<\/p>\n<p data-attrs=\"{&quot;url&quot;:&quot;https:\/\/saraszalmd.substack.com\/p\/your-labs-are-back-now-what?utm_source=substack&amp;utm_medium=email&amp;utm_content=share&amp;action=share&quot;,&quot;text&quot;:&quot;Share&quot;,&quot;action&quot;:null,&quot;class&quot;:null}\" data-component-name=\"ButtonCreateButton\" class=\"button-wrapper\"><span>Share<\/span><\/p>\n<p>Even after 30 years of practice as a gynecologist, this patient taught me something that my medical training didn\u2019t.<\/p>\n<p>A Female Edge reader, a clinician herself, described it in a comment this week: even after decades of practice, she is still learning something new every time a woman describes what\u2019s happening in her body in a way that doesn\u2019t fit the explanation she expected. <\/p>\n<p>Holding onto that humility, letting the patient in front of you pierce your default response, acknowledging that your training left things out: that\u2019s the real difference between a good clinician and a great one.<\/p>\n<p>My patient wasn\u2019t asking me what her AMH was. She was asking what it meant and how she should organize her life around it. She was handing me new information about what the normal reference range fails to capture: what it\u2019s like to be a woman her age, running out of certainty. I needed to be paying attention enough to receive it and respond in kind, with what we know and what we don\u2019t know. <\/p>\n<blockquote>\n<p><em><strong>Executive summary:<\/strong><span> <\/span><\/em><strong>Normal ranges answer population questions, not individual ones. The post teaches readers to (1) stop trusting a single snapshot and how to build a time series: same lab, same fasting state, same cycle day, three-to-six-month spacing, and (2) know what specific &#8220;next right step&#8221; looks like once a number crosses a line, without either panicking or dismissing it.<\/strong><\/p>\n<\/blockquote>\n<p>Here is what no one warns you about in your 20s, 30s, 40s, and older: you will spend a good chunk of your life being told your labs are normal by people who never asked what it feels like inside your particular body.<\/p>\n<p><span>Earlier this week, I offered <\/span>13 labs to print and discuss with your doctor<span>. This post is for the next appointment, the one where you discuss the lab results that land outside of where you hoped. I want to offer the deeper context about the optimal ranges instead of reference ranges and the part not enough doctor discuss with patients: what to do when the number crosses a line you are not about to interpret yourself.<\/span><\/p>\n<p>Routine tests are built to answer common clinical questions, and they do that well. The limitation is that any one result is a snapshot. A trajectory comes from repeated measures, symptoms, examination, family history, and the conditions under which a sample was collected.<\/p>\n<p><span>Another word for the trajectory is your trend over time, or to data scientists, a <\/span><em>time series.<\/em><span> I recommend that you build a time series with your labs that fall outside of the optimal range. You have to aim for the same conditions for each blood draw: get it at around the same time, after the same overnight fast (usually 12 hours), on the same day of your cycle, and with the same timing of your medications (if you\u2019re on any).<\/span><\/p>\n<p>A fasting glucose of 95 today tells you nothing about whether it was 82 last year or already climbing three points annually. <\/p>\n<p>An ApoB drawn once tells you where you stand, not where you\u2019re headed, as you can see from my recent ApoB time series (below).<\/p>\n<p>I have watched two patients present the same single number and the context makes the numbers mean two entirely different things: one stable for a decade, one accelerating toward a threshold neither of us saw coming because no one bothered to look.<\/p>\n<p>A time series only works if you hold the conditions steady enough to trust the comparison. Draw at the same time of day, the same fasting state, and for reproductive hormones, the same cycle day, since AMH, FSH, and estradiol move with your cycle in ways a single random draw cannot account for. Use the same lab when you can. Assay methods vary enough between companies that a shift from one lab to another can look like a trend when it\u2019s actually a different ruler. Space repeat draws three to six months apart for most markers, long enough for a real change to separate itself from ordinary biological noise, short enough to act before a slow drift becomes an emergency.<\/p>\n<p><em><strong>Bottom line: one lab value answers where you are. I also described ApoB in the post about <\/span>12 tests I run to check a woman\u2019s heart<span>.<\/span><\/p>\n<p><em>What that means in practice:<\/em><span> two women can share an identical LDL-C and carry very different ApoB, and it\u2019s ApoB, not LDL, that tracks with how many atherogenic particles are circulating. It\u2019s especially useful when triglycerides are high, metabolic disease is present, or LDL-C and non-HDL-C already look controlled but residual risk remains. this one is largely genetic. I draw it twice across a lifetime, not once. The first draw, in your twenties or thirties, captures your genetically fixed baseline. Estrogen suppresses the gene that produces it, and as estrogen falls during and after perimenopause, Lp(a) can rise, in some studies by close to a quarter compared to a woman\u2019s own premenopausal baseline. A second draw during or after perimenopause catches that shift. The 2026 ACC\/AHA dyslipidemia guideline recommends every adult receive at least one Lp(a) in their lifetime; I recommend two specifically for women, timed around that hormonal transition.<\/p>\n<p><em><strong>Bottom line: Two women with the same LDL-C can carry different risk. Most women who read that post and then get the actual result back are not prepared for how it feels to see one of those numbers on the wrong side of the line.<\/p>\n<p>Here is what I tell a patient in that exact appointment, the one where she is holding a lab report and asking me if her clock just sped up.<\/p>\n<p>A threshold crossed does not hand you a delivery date on when your fertility window closes. Age remains a stronger predictor of reproductive success than any one of these three numbers alone. [2]<\/p>\n<p>It does not measure egg quality. AMH estimates the size of the remaining follicle pool, not whether those follicles are chromosomally normal, a separate question age answers better than any blood test does.<\/p>\n<p>What it does do is change the urgency of a conversation you may have been planning to have later. If pregnancy is part of your future plans, that conversation moves up the calendar now, not next year.<\/p>\n<p>I have sat on both sides of this appointment, as the physician delivering the number and, once, as the patient receiving one that was not what I expected. Knowing the biology did not make the fifteen seconds after I saw my own result feel any less like the floor moving.<\/p>\n<p>The real question is not what the number means in a textbook. It is what you do in the appointment that comes after it, and that is exactly where most women are left standing alone.<\/p>\n<p><em>If you are between 20 and 45, this is the appointment you often cannot get: the one after the number, where someone explains what changes and what doesn\u2019t.<\/em><\/p>\n<p><em>Below: the decision framework I use the day a reserve marker crosses threshold, when I refer to a reproductive endocrinologist, and when I simply recheck in six months. The three hormones the last post didn\u2019t cover, testosterone, DHEA-S, and SHBG, and what each one adds once ovarian reserve is already in question. Direct answers to the two questions I\u2019m asked most in the comments: what a mildly elevated thyroid antibody means before it\u2019s Hashimoto\u2019s, and how to get a borderline result covered by insurance without it turning into unnecessary treatment.
